About the Opportunity

Reporting to the Manager of Seniors (Western Zone), or designate, the incumbent is a member of the Seniors LINCS Team and participates in the planning, development and provision of Social Work services to community-based seniors to enhance their independence and function at home. The Social Worker works to full scope of practice as an independent practitioner within the team. This includes supporting the development and integration of seniors’ health services within health care and the community and providing collaborative, client centered care across the continuum, supporting the psychosocial and emotional well-being of all individuals (includes patients, families, groups, populations and communities). This position includes considerable collaboration within the team and with community partners. The incumbent works with Primary Health Care one day per week, supervising BSW practitioners, running Community of Practice meetings, providing clinical supervision and case consultation weekly with Primary Health Care Social Workers.

The Social Worker ensures that the principles and practices that guide NSHA such as the mission, vision, values, expected behaviors, and the leadership philosophy along with organizational health, population health, ethics, safety, quality, partnerships and interprofessional collaboration are integrated within the services one provides and through the messages one delivers.

About You

We would love to hear from you if you have the following:

  • Master's of Social Work required
  • Certificate of Registration and Membership in the Nova Scotia Association of Social Workers and Canadian Association of Social Workers.
  • Minimum two (2) years clinical experience required
  • Experience in Geriatric Care preferred
  • First Aid/CPR
  • Strong counselling and communication skills
  • Capacity to achieve a high degree of autonomous practice
  • Ability to work within a multidisciplinary team context
  • Knowledge of issues related to family violence, sexual abuse, addictions, grief and loss, end of life, disability, crisis intervention, caregiving and continuum of care and intervention skills that come from knowledge of theoretical frameworks/approaches to practice
  • Competencies in other languages an asset, French preferred.

  • Permanent Part-Time, 60 Hours Biweekly

Compensation and Benefits

$37.31 - $49.05 Hourly
