About the Opportunity
The Diabetes Centre is a community-based program that provides patients, and their families, with the support they need to manage their Type 1 and Type 2 diabetes. The goal of the DMC is to provide education and self-management support for patients to manage their blood sugars, cholesterol levels, and blood pressure. The Diabetes Centres assist patients to maintain their health and prevent the complications of diabetes in a model that follows individual and group programming, both in person and virtually. The Diabetes Centres support an interdisciplinary and integrated approach to diabetes education. Patient-centred care, behaviour change, and self-management are core to all programming and services.
Reporting to the Health Services Manager, Primary Health Care, the Interdisciplinary Clinical Leader provides leadership, innovation and support to the clinical team and supports the development, implementation and coordination of Diabetes Centre services across multiple sites within the Central Zone. This includes monitoring patient flow, scheduling and supporting a variety of quality improvement and evaluation activities, and triage of referrals. The Interdisciplinary Clinical Leader acts as a role model and exhibits team building and leadership skills. She/he demonstrates effective communication and organizational skills and works with the health care team, community groups and agencies, other providers, and partners to ensure consistent, equitable, timely and coordinated care is provided to clients. The incumbent is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities. Excellent interpersonal and written communication skills are required. The Interdisciplinary Clinical Leader will develop close working relationships and extensive collaboration with a multitude of colleagues and intersectoral partners including Endocrinology, Diabetes Care Program of Nova Scotia, Primary Care Providers, community-based organizations, provincial program partners and other stakeholders. The Team Lead applies knowledge, skills, and professional judgment in planning and facilitating change through processes such as partnership engagement, collaborative interdisciplinary approaches to care, assessment of community needs and community capacity. She/he supports program evaluation, the use of outcome measures, continuous quality improvement, and works with Dalhousie School of Health Professions to support educational opportunities for students.
About You
We would love to hear from you if you have the following:
- Baccalaureate degree in Human Nutrition, Pharmacy or Nursing plus 5 years' related experience or Master's degree plus 3 years' related experience
- Licensed with the respective Nova Scotia regulatory college
- Current status as a Certified Diabetes Educator preferred (or willingness to obtain within two years of hire)
- Experience working in a Primary Health Care setting/community-based team required
- Previous experience in team development and leadership working within a collaborative interdisciplinary team
- Individual and group education/facilitation skills utilizing adult education, self-management support and/or behaviour change principles
- Understanding of population health, health promotion strategies, chronic disease prevention and management and behaviour change principles is an asset
- Problem solving ability and willingness to considered uncharted or novel ideas related to trends in healthcare
- Coaching ability and diplomacy when dealing with co-workers, clients and community members
- Ability to establish and maintain good working relationships with clients, colleagues, management and community agencies
- Superior organizational and time management skills
- Ability to work independently and collaboratively as part of an interdisciplinary team
- Required travel in the community; Valid driver's license and access to a reliable car for travel
- Competencies in other languages an asset, French preferred

Compensation and Benefits
$40.99 - $51.17 hourly
