About the Opportunity

Reporting to the Manager of Community Transitions the Social Worker provides assessment, consultation and collaborates with the interdisciplinary team to provide client and family centered care planning and interventions. The Social Worker also collaborates with the outreach team members and community partners to assist with system navigation, advocacy and resource location. They will also work closely with team members to continue to develop resource directory.

This position is based out of West Bedford Transitional Health (WBTH) – Hogan site but will work within the community, providing services to clients throughout Central Zone who have transitioned from WBTH. The successful incumbent for this position will specialize in providing the following support to clients:

  • Housing - liaise with in house (Shannex team) to gain priority access to housing, refer to by-name list, liaise with community partners when clients are housing insecure. The social worker will build rapport and communication links to community resources for clients who are recently housing and food insecure.
  • Financial - Assist clients who are experiencing difficulties with access to drug/ medication programs, Pharmacare advocacy, sleep machine equipment and funding, other funding programs not related to TESS and CESS, financial supports for medical supplies, deep cleans to homes etc. Assist clients after transition from WBTH with applications for programs such as I.E, CPP, GIS, OAS, CPP-D.
  • Family/Care Partner Support - Along with interdisciplinary team members and in close collaboration with BRC the SW will identify care partner stress and offer support and resource location. Involved in the development of new pathways and approaches for supporting family and care partners.

About You

We would love to hear from you if you have the following:

  • Bachelor of Social Work
  • Registered with the Nova Scotia College of Social Workers (NSCSW)
  • Minimum two (2) years’ experience as a Social Worker
  • Experience with community and/or care of the elderly is preferred
  • Maintain current CPR/RQI or BLS-C Certification
  • Expert knowledge of resources
  • Proficiency in understanding organizational and program policies and procedures
  • Ability to establish partnerships and develop collaborative relationships with key stakeholders both within the organization and community
  • Demonstrated effective communication and interpersonal skills
  • Valid Nova Scotia driver’s license and access to a reliable vehicle
  • Competencies in other languages an asset, French preferred

Hours

  • Permanent Full Time; 75 hours bi-weekly
  • Currently the model of work is 5 days/ week, but this may change in the future to a 7-day working model
  • Some opportunity to work from home may be available

Compensation and Benefits

$36.58 - $47.35 hourly
